Box Breathing is a structured breathing technique built around four equal phases: inhale, hold, exhale and hold again. A common pattern is inhale for 4 seconds → hold for 4 seconds → exhale for 4 seconds → hold for 4 seconds.
The four equal phases are what give the technique its name. If you imagine each phase as one side of a square, a complete breathing cycle traces the four sides of a box. Unlike Equal Breathing, which simply balances inhalation and exhalation, Box Breathing introduces deliberate pauses after both. This makes it more structured and gives your attention something specific to follow throughout the entire breathing cycle.
It's often associated with maintaining composure under pressure, but there's nothing inherently performance-focused about it. At its simplest, Box Breathing is a memorable way of slowing and structuring your breathing.
Why does Box Breathing work?
Box Breathing combines several elements that can influence how the exercise feels: slower breathing, a predictable rhythm, deliberate breath holds and sustained attention to the breathing cycle.
Slow breathing can affect the interaction between respiration and cardiovascular activity. Heart rate naturally changes across the breathing cycle, while slower breathing can increase respiratory sinus arrhythmia and some measures of heart rate variability. The counting itself may also be useful. Following four distinct phases gives your attention a simple, repetitive task, so if your thoughts are moving quickly or you're dealing with a demanding situation, concentrating on inhale → hold → exhale → hold can provide a temporary attentional anchor.
The breath holds distinguish Box Breathing from simpler paced techniques. They interrupt the continuous movement between inhalation and exhalation and create a highly defined respiratory pattern. However, claims about Box Breathing should be kept proportionate to the evidence. Slow breathing has a substantial physiological research base, but Box Breathing as a specific four-phase protocol has been studied much less extensively. We shouldn't assume that every effect observed in slow-breathing research is caused specifically by the box pattern or its breath holds.
How to do Box Breathing
Sit or lie somewhere comfortable and allow your normal breathing to settle before beginning.
- Breathe in gently through your nose for 4 seconds.
- Hold your breath comfortably for 4 seconds.
- Breathe out slowly for 4 seconds.
- Pause after the exhale for 4 seconds.
- Begin the next inhale and repeat.
Continue for around three to five minutes. Four seconds per phase is a convenient starting point, not a requirement. If four-second holds feel uncomfortable, try three seconds instead. The important feature is the equal four-part structure, not achieving a particular duration.
What should it feel like?
Box Breathing should feel controlled and predictable rather than demanding. The holds should feel like comfortable pauses; you shouldn't be fighting the urge to breathe before the count finishes.
Your inhale should remain gentle rather than becoming a large preparatory breath designed to get you through the upcoming hold. Similarly, you don't need to force all the air from your lungs before the second pause. Ideally, each phase flows calmly into the next: inhale → pause → exhale → pause. If you're repeatedly gasping when the next inhale begins, reduce the count.
Common mistakes
- Making the breath holds a challenge. The objective isn't to prove that you can hold your breath for four, five or six seconds. If the holds create significant air hunger, shorten the entire pattern.
- Taking a huge inhale. Knowing that a breath hold is coming can encourage you to fill your lungs as much as possible. This isn't necessary; keep the inhale comfortable.
- Forcing the exhale. You don't need to completely empty your lungs before the second hold. A controlled, relaxed exhale is enough.
- Increasing the count too quickly. Longer isn't automatically better. A comfortable 3:3:3:3 pattern is more useful than struggling through 6:6:6:6.
- Losing the four-phase structure. It's easy to shorten one phase unconsciously, particularly the pause after exhaling. Following a visual or audio pacer can help keep the four sides approximately equal.
When should you use Box Breathing?
Box Breathing can be particularly useful when you want a breathing exercise that demands enough attention to interrupt what you were doing previously. That can make it suitable before a demanding task, presentation, difficult conversation or other situation where you want to move from feeling scattered or pressured towards feeling more composed.
The four-phase count also gives it more cognitive structure than techniques such as Diaphragmatic Breathing or Extended Exhale. If you find that your attention wanders during simpler breathing exercises, having something to track throughout the entire cycle may be helpful. It can also work as a short transition between activities, with a few minutes of deliberately structured breathing creating a clear boundary between what you were doing and what you're about to do next.
When might Box Breathing not be the best choice?
Box Breathing isn't necessarily the best introduction to breathwork, particularly if deliberate breath holds feel uncomfortable. The most appropriate alternative depends on what you're trying to achieve:
- If you want a simpler counted rhythm: Equal Breathing provides a similar structure without the breath holds.
- If you primarily want to slow down and unwind: Extended Exhale maintains a continuous breathing cycle while giving more time to the exhale.
- If you want sustained slow breathing: Resonant Breathing is more directly focused on breathing frequency and its relationship with cardiovascular rhythms.
- If your main difficulty is maintaining attention: Breath Counting provides an attentional anchor without requiring you to manipulate every phase of your breathing.
The technique should match both the desired outcome and the person practising it. Some people enjoy the precision of Box Breathing; others find breath holds unnecessarily restrictive.
Box Breathing vs Equal Breathing
Box Breathing and Equal Breathing both use symmetry, but they structure the breathing cycle differently. Equal Breathing typically uses inhale 4 seconds → exhale 4 seconds, whereas Box Breathing adds two additional phases: inhale 4 seconds → hold 4 seconds → exhale 4 seconds → hold 4 seconds.
Equal Breathing therefore provides a continuous two-phase rhythm, while Box Breathing divides the cycle into four clearly defined sections. That makes Equal Breathing simpler and potentially more accessible when you're learning paced breathing. Box Breathing requires more attention and introduces the additional sensation of breath retention.
If you want a gentle, balanced breathing rhythm, Equal Breathing may make more sense. If you want a highly structured exercise that occupies your attention and gives you a clear sequence to follow, Box Breathing may be more appropriate. Neither is inherently superior; they use different structures for different purposes.
Related breathing techniques
- Equal Breathing: uses the same idea of symmetry but removes the breath holds. It's a useful alternative if you like the predictable rhythm of Box Breathing but want a simpler, continuous breathing cycle.
- 4-7-8 Breathing: also incorporates breath retention, but the phases deliberately have different lengths. It's more asymmetrical than Box Breathing and places much greater emphasis on the exhale.
- Breath Counting: provides structure for attention without imposing a breathing rhythm. It may be useful if the counting aspect of Box Breathing helps you focus but deliberately controlling every phase of the breath doesn't.
- Resonant Breathing: removes the holds and focuses instead on maintaining a slow, continuous breathing frequency. It's particularly relevant when the aim is sustained slow breathing rather than following a four-stage sequence.
What does the science say?
Box Breathing presents an interesting evidence problem because the technique is widely discussed, while research examining the exact equal inhale → hold → exhale → hold protocol is relatively limited. There is considerably stronger evidence for some of its components, particularly slow-paced breathing.
A systematic review by Zaccaro and colleagues examined the psychophysiological effects of slow breathing. Across the included studies, slow breathing was associated with changes in cardiovascular and autonomic measures including heart rate variability and respiratory sinus arrhythmia, alongside reported psychological effects such as increased relaxation and reduced arousal. However, the breathing protocols varied substantially, so the review supports slow breathing generally rather than Box Breathing specifically.
Research by Balban and colleagues provides a more direct comparison. In a randomised controlled study, participants practised several short daily breathing exercises, including cyclic sighing, Box Breathing and cyclic hyperventilation with retention, or mindfulness meditation. The breathwork groups showed improvements in mood and reductions in respiratory rate, with cyclic sighing producing the strongest improvement in positive affect within the breathing groups. Box Breathing therefore showed benefits within the study, but it wasn't superior across all outcomes, and the research doesn't establish that the four equal phases have unique therapeutic properties.
The breath-retention component also deserves some caution. Breath holding changes blood gases and cardiovascular responses, particularly as the hold becomes longer. A comfortable four-second pause is very different from prolonged breath-hold training, so research into extended apnoea shouldn't be applied directly to ordinary Box Breathing.
Overall, the most defensible interpretation is that Box Breathing provides a convenient structure for combining slow breathing, attention and brief breath retention. Slow breathing itself has a reasonable evidence base, while evidence for additional benefits specifically created by the box pattern remains more limited.

These studies provide useful evidence for the components underlying Box Breathing and, in the Balban study, direct investigation of Box Breathing itself. The current evidence is stronger for the general effects of controlled slow breathing than for claims that the specific four-sided pattern has unique physiological benefits.
