Diaphragmatic Breathing is less about following a particular breathing pattern and more about how the breath moves through your body.
The diaphragm is a dome-shaped muscle beneath the lungs and the primary muscle involved in breathing. When you inhale, it contracts and moves downwards, helping create the pressure change that draws air into the lungs. As this happens, the abdomen naturally moves outwards. When you exhale, the diaphragm relaxes and the abdomen moves back in.
This is why Diaphragmatic Breathing is sometimes called “belly breathing”, although that name can be slightly misleading. You aren't actually breathing into your stomach. The movement you see in the abdomen is a consequence of the diaphragm doing its work.
Unlike Equal Breathing or Extended Exhale, Diaphragmatic Breathing doesn't require a particular count. The emphasis is on allowing the breath to remain comfortable while becoming more aware of the mechanics behind it.
Why does Diaphragmatic Breathing work?
The diaphragm normally plays a central role in quiet breathing. But breathing patterns can change with posture, physical exertion, respiratory conditions, stress and other factors, and some people notice more movement and tension around the upper chest, neck and shoulders.
Diaphragmatic Breathing deliberately brings attention towards the movement created by the diaphragm. Rather than trying to pull in the largest possible breath, the exercise encourages gentle abdominal expansion while limiting unnecessary movement and tension in the upper body.
Research into diaphragmatic breathing has investigated several possible effects, including changes in respiratory rate, physiological stress markers and subjective measures of stress and anxiety. There is some encouraging evidence, although the studies vary considerably in their populations, methods and definitions of diaphragmatic breathing.
There's also a practical reason to learn it. Many other breathing exercises become easier once you can breathe comfortably without repeatedly lifting your shoulders or forcing large breaths into the upper chest.
In that sense, Diaphragmatic Breathing can be thought of less as a specialised breathing pattern and more as a useful foundation for other techniques.
How to do Diaphragmatic Breathing
When you're first learning the technique, lying down can make it easier to notice the movement of your abdomen. Once the pattern feels familiar, you can practise while sitting or standing.
- Lie comfortably on your back with your knees slightly bent, or sit somewhere with your back supported.
- Place one hand lightly on your upper chest and the other on your abdomen, just below your rib cage.
- Breathe in gently through your nose.
- Allow your abdomen to rise beneath your lower hand as you inhale.
- Keep the movement in your upper chest relatively quiet rather than deliberately lifting your shoulders.
- Exhale gently and allow your abdomen to fall naturally.
- Continue for around five minutes.
There's no need to introduce a particular inhale-to-exhale ratio when you're learning. Let the breath determine its own comfortable pace.
Once diaphragmatic movement feels natural, you can combine it with paced techniques such as Equal Breathing or Extended Exhale.
What should it feel like?
The movement should be gentle.
As you inhale, you should notice the hand on your abdomen move outwards while the hand on your upper chest remains relatively still. Some chest movement is normal; the goal isn't to immobilise your rib cage.
You also shouldn't deliberately push your stomach out. The abdominal movement should happen as a consequence of inhaling rather than becoming a separate muscular action.
Your shoulders and neck should remain relaxed, and the breath shouldn't feel unusually large.
If you're working hard to make your abdomen move, reduce the effort and return to normal breathing before trying again.
Common mistakes
Trying to breathe into your stomach
The term “belly breathing” can create the impression that air is somehow entering the abdomen. It isn't. Air remains in the lungs; the abdominal movement occurs because of the movement of the diaphragm and abdominal contents.
Pushing the abdomen out
You don't need to deliberately extend your stomach during the inhale. Allow the movement to happen rather than manufacturing it.
Taking enormous breaths
Diaphragmatic Breathing doesn't mean filling your lungs as much as possible with every breath. Large, forced breaths can make the exercise uncomfortable and may lead to light-headedness.
Trying to eliminate all chest movement
The rib cage naturally participates in breathing. The goal is relaxed diaphragmatic breathing, not keeping the chest completely stationary.
Adding complicated timings too early
If you're simultaneously trying to move your diaphragm correctly, breathe for exactly five seconds and extend your exhale, the exercise can quickly become unnecessarily complicated.
Learn the movement first. Timing can come later.
When should you use Diaphragmatic Breathing?
Diaphragmatic Breathing can be particularly useful when you're learning breathwork for the first time or notice that breathing exercises tend to create tension in your shoulders, neck or upper chest.
It can also work as a short standalone practice when you want to bring attention back to the physical sensation of breathing without following a complicated sequence.
Perhaps its most useful role, however, is as a foundation.
Once comfortable diaphragmatic breathing becomes familiar, you can apply the same mechanics while practising Equal Breathing, Extended Exhale, Resonant Breathing and other paced techniques.
That means Diaphragmatic Breathing doesn't necessarily need to compete with those techniques. It can sit underneath them.
When might Diaphragmatic Breathing not be the best choice?
If you're already breathing comfortably from the diaphragm, repeatedly concentrating on abdominal movement may not add much to a particular session.
If your main problem is that your thoughts are racing or your attention keeps wandering, Breath Counting may give your mind a more useful task.
If you're specifically trying to establish a slower breathing frequency, Resonant Breathing provides a clearer rhythmic target.
And if you're looking for a structured breathing pattern to use during a short period of pressure or preparation, Box Breathing may provide more active guidance than simply observing diaphragmatic movement.
There are also situations where breathing difficulty shouldn't simply be treated as a breathing-practice problem. New, severe or unexplained shortness of breath warrants appropriate medical assessment rather than attempting to correct it with a breathing exercise.
Diaphragmatic Breathing vs Breath Counting
Diaphragmatic Breathing and Breath Counting make a useful comparison because they focus on two very different parts of the breathing experience.
Diaphragmatic Breathing → how you're breathing
Breath Counting → where you're placing your attention
With Diaphragmatic Breathing, you pay attention to the physical mechanics of the breath: the movement of the diaphragm, abdomen and upper body.
With Breath Counting, you generally leave those mechanics alone. Instead, you count successive breaths and use the count as an anchor for attention.
If your breathing feels tense or you find yourself repeatedly lifting your shoulders when trying breathing exercises, learning comfortable diaphragmatic movement may be the better starting point.
If your breathing already feels comfortable but your attention keeps drifting, Breath Counting may make more sense.
They're also easy to combine. Once you've established relaxed diaphragmatic breathing, you can count those breaths without changing their natural rhythm.

Breath Counting shifts the focus away from breathing mechanics and towards attention. It's a natural next step if you're comfortable with the physical breath but want something to help keep your mind anchored to it.
Pursed Lip Breathing
Pursed Lip Breathing changes how you exhale by breathing out through narrowed lips. It has particularly important applications in respiratory rehabilitation and is therefore quite different from general relaxation-focused breathing exercises.
Equal Breathing
Equal Breathing introduces timing while retaining a relatively simple breathing cycle. Once diaphragmatic movement feels comfortable, an equal inhale and exhale can add a predictable rhythm.
Resonant Breathing
Resonant Breathing combines comfortable breathing mechanics with a deliberately slow breathing frequency. It can be useful if you want to explore the interaction between respiration and cardiovascular rhythms.
What does the science say?
Diaphragmatic Breathing has been studied in several contexts, although the evidence base is heterogeneous. Different studies use different breathing protocols, practice durations and participant groups, making it difficult to treat “diaphragmatic breathing” as a single standardised intervention.
A 2017 study by Ma and colleagues randomly assigned 40 healthy participants either to an eight-week diaphragmatic breathing intervention or a control group. The breathing group showed improvements in sustained attention, reductions in self-reported negative affect and lower cortisol concentrations following training. The study is frequently cited in discussions of diaphragmatic breathing, although its relatively small sample means its findings shouldn't be generalised too broadly.
A systematic review by Hopper and colleagues examined diaphragmatic breathing as a potential intervention for stress reduction in adults. The included studies generally reported improvements in physiological or psychological stress measures, but the authors also highlighted substantial differences between the interventions and methodological limitations in the available research.
Diaphragmatic breathing also appears within respiratory rehabilitation. A Cochrane review examined breathing exercises for people with chronic obstructive pulmonary disease (COPD), including diaphragmatic breathing, pursed-lip breathing and other approaches. Some breathing exercises improved functional exercise capacity, but effects varied between techniques and outcomes. Importantly, evidence from people with respiratory disease shouldn't automatically be applied to healthy populations practising breathwork for relaxation.
The broader slow-breathing literature provides additional context. Slow breathing has been associated with changes in heart-rate variability, respiratory sinus arrhythmia and autonomic cardiovascular regulation, but these effects involve breathing frequency as well as mechanics. They therefore can't be attributed specifically to diaphragmatic movement.
Overall, there is evidence suggesting that deliberate diaphragmatic breathing can influence psychological and physiological outcomes, but the research doesn't justify treating it as a universal solution for stress or dysfunctional breathing. Its clearest practical value may be more fundamental: learning to breathe comfortably using the body's primary respiratory muscle provides a useful basis for many other breathing practices.

These studies examine Diaphragmatic Breathing in different populations and contexts. Evidence from clinical respiratory rehabilitation shouldn't be interpreted as demonstrating the same effects in otherwise healthy people practising breathing exercises for wellbeing.
